It's kind of wild that this works, but it does. After a few pumps, my cock hits the end of the tube, and after holding the vacuum for 15-20 min, it stays noticeably plumper for a couple of hours. Piece of advice: the silicone sleeve is very soft and sticky, so a good, water-based, anal lube is better to keep it from being painful to put on.
